The highly anticipated new Axis Community Health integrated care facility in Livermore is officially open to patients, bringing primary medical care, dental services, behavioral health support and enrollment assistance under one roof.
Axis is a nonprofit healthcare provider that offers affordable services for uninsured and low-income residents in the Tri-Valley. The new state-of-the-art clinic, which opened on Jan. 6, is Axis’ sixth location and first comprehensive health care hub, officials said in an announcement.
Located at 1686 Second St., the 8,800-square-foot clinic takes over a building previously occupied by Legacy Real Estate & Associates. A groundbreaking ceremony in the summer of 2023 kicked off renovations to transform the space into the care facility, which included establishing six fully equipped dental operatories featuring the latest technology, eight modern medical examination rooms and three specialized mental health counseling spaces.
With a projected annual capacity of 18,000 total patient visits, the clinic will provide an estimated 9,000 medical appointments, 6,800 dental procedures and 2,200 mental health consultations, according to officials.
Axis patients also have access to care coordinators who can assess their needs and connect them to resources like healthy meals and transportation to appointments.

“This integrated model allows Axis’s medical, dental, and behavioral health teams to collaborate seamlessly, resulting in better outcomes for patients. By bringing these multiple services under one roof, we are able to address a patient’s whole-person health,” Axis CEO Liz Perez-Howe said.
With all existing Axis sites operating at full capacity, the need for expansion was evident. In terms of deciding the best place for the new clinic, officials said that Axis community health data shows that 39% of their patients reside in Livermore, making it their most populated patient area.
“This expansion represents more than just a new building – it’s a promise to keep our community healthy and thriving,” Perez-Howe said.
