Alta Bates fined for surgery mishap

The Alta Bates violation stemmed from an incident last December, when a 79-year-old patient went in for surgery to repair a heart valve. During the surgery, a ring sizer - a disc about the size of a quarter, used to measure the valve opening during repair - was left in the sac that encloses the heart.
The ring sizer was taken out about two weeks after the initial operation, and required major open-heart surgery to remove. The patient need a tracheotomy after the surgery and went into kidney failure.
